We put a great deal of time and effort into ensuring that you will receive a healthy, happy, socialized family addition. Have you owned a Dog before? If so, what breeds have you owned? Do you still have them? If not, what happened to them?

What role will your new Dog play in your family? Will he be strictly a companion, or do you plan to compete in Conformation or some form of dog sport? Do you plan to breed your dog?

How many are in your family? Please list the ages of all.

Do you have a fenced yard?

Do you work outside the home? Please give the approximate hours per day the puppy will be home alone.

Please list your Veterinarians name and phone number.
